2023 Measurement Year Registration Addendum for Other Providers (Optional)

The instructions in this addendum are intended for clinics that wish to use MNCM’s registration process to register providers, that fall outside of the SQRMS registration mandate, for the purposes of federal or other reporting requirements.  THIS IS AN OPTIONAL REGISTRATION STEP.
