MNCM advises the following:
- Exclude encounters that occurred outside the submitting
medical group, but include data obtained from these encounters if it
resides in the submitting medical group’s patient record (ex: blood pressures documented in the patient BP flowsheet, diagnoses that are added to the patient's active problem list, medications added to the active medication list, etc.,.).
- Encounters that occurred within the submitting medical
group at a location/clinic that doesn’t qualify for mandated reporting (i.e., NOT located in MN or does NOT have an MD or DO providing patient care) can
be registered or submitted (optional) for numerator data (i.e., weight,
height, tobacco status, etc.).
- If you choose to register the clinic (optional), then
you can submit the clinic ID in the encounter data file.
- If you choose to not register the clinic (again
optional), then you can submit these encounters with a blank clinic ID.
- Both options are only acceptable if the data obtained
from these encounters can be verified in the patient’s clinical record
during audit.
- Most of the time it is easier to register sites that
aren’t required to ensure numerator data is maximized.
