The 2020 HIT survey is now open
The Health Information Technology Survey is now open!
All Minnesota physician clinics must complete the 2020 HIT Ambulatory Clinic Survey between September 15, 2020 through October 15, 2020.
Please note that due to the survey settings, the survey works best if completed in one setting. To reduce time, please use the attached PDF survey tool to collect the survey answers before accessing the survey.

Respondents who reply for multiple clinic sites that all operate from the same EHR system and have the same processes of care may request that responses from the survey entry be duplicated to these other clinics. A request to duplicate responses may be submitted at the end of the web survey.
If you have multiple EHR platforms across your medical group, the survey should be taken once for each unique platform. Note that if you do not currently have an EHR, the web survey will skip you from question 5 to question 36.
The person completing the survey should meet the following three criteria:
- Work at least part-time at the physical clinic location,
- Be familiar with the clinic’s health information technology/EHR system, and
- Have experience with the clinic’s operations.
- Prior experience has shown that involving your IT department (if applicable) in reporting your EHR’s total functionality increases the accuracy of the survey data.
Results from the survey are used by MDH to report the status and use of electronic health records, health information exchange and other health information technology across Minnesota. Prior to any results being publicly reported by MNCM, a preview period will be conducted and your survey results will be forwarded to you.
Your clinic may also be selected for a random validation audit of your survey responses later in April. If selected, MNCM will notify you of this process and timeline.
