SS&C Health / Credo / ChartFast: Chart Review Vendor for DHS/MNCM Health Care Disparities Report
MN Community Measurement (MNCM) has contracted with
the vendor SS&C Health, to conduct chart reviews as part of the Health Care Disparities Report funded by the Minnesota
Department of Human Services (DHS). DHS is required to conduct this study
because of legislative action (Minnesota statute 256B.072).
Medical groups may receive chart requests from SS&C Health's chart retrieval vendor, Credo / ChartFast. These requests will generally be received between April and June. The vendor is requesting charts on patients enrolled in Minnesota Health Care Programs
for the Controlling High Blood Pressure HEDIS measure that is included in the report. Please see the attached memo for further detail. Upon request, the employee from Credo / ChartFast that is submitting the chart retrieval request should also be able to provide this letter as evidence of their participation in this project.
