Please use the template attached to send us a list of all medications and allergies that your organization believes should receive numerator credit for the aspirin/anticoagulant/antiplatelet and statin components for diabetes and vascular measures.
Please note that only medications and allergies that would receive numerator credit need to be crossmapped.
